There's far too many to mention. The UK's NHS sends people abroad for treatment, to the USA and Europe. Either it doesn't offer the treatment itself, or it can be provided sooner. No need for insurance companies in this one because the NHS pays IF it considers the treatment medically necessary.
A lot more private treatment sought abroad these days. Dentistry, both necessary and cosmetic. Cosmetic surgery. BIG ethical issues if things go wrong!! A UK woman died recently in a Turkish private clinic having her bum done. Was the clinic officially "recommended"? Whose law applies? If there's a rights issue involved, can I she in the European courts?
What might insurance insure? Accidental health matters, yes. Foreseeable existing problems, yes. Chosen cosmetic treatments? Unlikely. Could I buy my insurance in a third country? IE travel from the UK, require treatment in Chile, on insurance purchased in France? Probably not, but that restricts the insurance market against me.

Medical tourism is promoted based on commission offered. One who is struggling with health should not be charged as commission and human right is violated to have healthy life
