What are the most common HSSE related human failures in the onshore and offshore Oil and Gas Industry?

I am just researching any input from your side is welcome.

There are several Nishit, but must remember that these failures are vary from operation to operation at on and offshore domains. In my personal opinion during performing extraction and production activities chances of human error are high due to the complexity of this job.
