It is mainly related to insulation failures leading to short circuit. The insulation fails due to ageing and degradation caused by electrical, mechanical and thermal stresses all playing some role. However, the most significant are the electrical stresses causing partial discharges and thermal stress causing overheating and insulation breakdown.
Insulation failure is the correct & the most catastrophic result when related failures such as bearing or over load generate abnormal heat to the insulation system. The insulation failure generates internal winding, auto-Transformer effects. Auto-transformer effects within the windings can result in a slow overheating or a rapid high energy short circuit. Best Practice in my opinion to mitigate the resulting hazardous conditions is two fold. 1) Thermal Detecting Systems and 2) Over Current Protection.
