Despite of the big quantity of research initiatives worldwide, CR as contended by Mitola or Haykin is still far away from mobile devices. I think a big impediment for CR adoption worldwide is the lobby of TV and other primary users. What do you think?
Recently, the FCC opened up TV white spaces for secondary users in the US. I think they took the approach that no spectrum sensing is used, you rather query a database with all primary users to find out if you are allowed to send.
In that sense "cognitive" is not really required (spectrum agility, fast sensing to detect spectrum holes, etc.) to achieve good performance. There are lots of problems with interoperability in CR, sometimes ideas start to have lots of practical issues when applied, and I guess CR went down that path.

I think CR technology is yet in its infancy and needs a lot of improvement and research. it is an open research challenge for the researchers , the way to improve spectrum sensing and other parameters is required. but once implemented, CR along with SDR (software defined radios) may revolutionize the wireless industry and open up many new service areas.
I think that another big issue is related to spectrum trading. The IEEE is now making a revision of the standard about dynamic spectrum access (DSA), and one of the main issue to be answered is about the secondary market, spectrum trading and second operators' revenues.
