I am looking for a plot library in C++ that can be used for data visualization mostly of radio signal visualization including power spectral denstiy, FFT, time domain signal, scatter plot. Data is stream processed in real time in a C++ windows form application written in visual studio C++ or QT environment.
Please assert if your answer is for stream data, batch data, or both ?

I'd personally go with Qwt.
Or switch your GUI code to Python (you can still use Qt through PyQt) and use matplotlib, although that might not be fast enough for realtime. Nonetheless, there is still PyQwt for the realtime processing case.
Have a look on VTK - http://www.vtk.org/ it may be to complex for your problems, but should cover them completely.
We have used GLG Toolkit from Genlogic successfully for both real-time monitoring and offline charts of complex SCADA data. Has C++, Qt and .net variants available for free for research purposes.
OxPlot is another great MIT licensed library with many features such as direct export to PDF, SVG etc. and works well .Net, WPF and many similar technologies. Works great for realtime charts as well as offline charts.

since long time passed from this question i would like to report my usage upon some of the people asking. actually QTchart is the king of C++ charting however its commercial. but I have used QCustomPlot which is number 2 in QT platform for my charting purpose. its good enough but as perfect as QTchart.! you can be hopeful to see QTchart inGPL license in the next version of QT, however it is not available free of charge currently yet.
Hi kindly suggest me Real-time graph and chart plotting library , with win 32 application using c++. Qt chart is not useful in my case. If anyone have any idea, Please share.
Did anyone suggest DISLIN already? It is incely integratable in c++ and MFC even with a static link ... not DLL hell.

This is a cross-platform solution, it can be called from C/C++ code and it produces publish-ready, adjustable plots that can be imported into latex documents for example.
I've been building https://github.com/thclark/cpplot which allows you to generate most of the figure types that the plot.ly library is capable of, straight from c++.
It's early days, but there's a good cross-platform build system, documentation, and a browser utility for figure viewing. It's compatible with eigen, which is helpful for numerics people, and it's open source under the MIT license, so doesn't require your code to be open sourced too.
Resulting figures can then be viewed in the browser, where you can export for publications etc. They can be used with plotly online too, if that's your thing.
morphologica provides modern OpenGL based plotting and data visualization for C++:

It's so fast that you can visualize data without using much CPU resource. I use it to visualize computational simulations.

There is probably not a better one but there are some with better defined mathematical models that do their modelling better.
