In stable ELBW , and for reducing phlebotomy & pain , we just check BS test & ABG as necessary , No routine tests up to end of 3-7 days ! We monitor IVT according clinical evaluation , weighting twice daily , I&O , urine markers ,.....
Yes, I have the same policy. But, when I submitted a case report the reviewer comment that we have not check the electrolytes frequent enough in the first 24 hours although we had two tests in the first 12 hours.
we check TSB,Lytes, once daily until day 5 (after which fluids wont be increased ) ,blood sugar twice daily with blood gas. unless there was abnormalities.
if you do this more frequently I suggest to review whether this practice was of benefit for your babies or not .
I am doing the same thing. But in one of my case report manuscript the reviewer criticized me not having electrolytes check every 3-4 hours in the first day of life when the microprimie was extremely unstable with severe hypotension and pulmonary hypertension. I thought I did something wrong and would like to have inputs from somebody in the field.
