I'm partial to Lorna Benjamin's Interpersonal Approach, as detailed in
Interpersonal Diagnosis and Treatment of Personality Disorders: Second Edition (Guilford, 2002). I wouldn't get too hung up about "valid treatments" and be open to the wide range of treatment approaches, and to simply understanding the disorder.

Thanks for your message. Yes, the closet/stealth narcissist and the covert narcissist are synonymous to each other. According to some, it is more difficult to recognize covert narcissists than the overt ones. Please see the following links.

Not usually, but some of them came in for depression or anxiety or relationship problems. They didn't come in with full-blown personality disorders, but I'd discern some narcissistic traits in them after having worked with them for a while and/or together with their significant others.
According to my experience their significant other can not express her (80%) freely and understandable in these situations. She usually seems to be the one to blame. At least she is the one containing the NPD:s distress.
Yes, the interpersonal dynamics you described could indeed play out in therapy. Sometimes we miss them.
Yet overt or even covert narcissists would somehow show their personality features as we come to know them over time. The treatment of NPD is still very challenging.
