We have been studying that since 1998. In 1999, I started to try to publish a study I just posted on my profile. Last year I tried once again to send it to a phlebology congress with no success. The outcomes of the study are the first seeds of the CLaCS technique (Cryo-Laser and Cryo-Sclerotherapy guided by Augmented Reality). about that has been always rejected since 1999. I believe the most problem is that your study is not comparative. I do a lot of sclerotherapy in C1 and the most problem is hyperpigmentation. If you have only 1,5% of it maybe it would be very interesting to compare this rate with just a sclerotherapy in prospective study.
I'm also belive that the undoubtful advantage of just a sclerotherapy is a cost of prosedure. If we add to the treatment something that increase the cost we should have really strong evidences of better results. Maybe comparing of something like patients opinion about improvement with the visual-analogue scale would be of value.

I've been doing CLaCS guided by Augmented Reality since May 2005. I've been developing the method of combining light and sugar since 1995. I started combining existing methods that had poor literature but were safe, and the results were really good. My clinic is private and we did some small analysis. In 2005, I had already very good results and I had not anymore "equipoise" to random my private patients to an arm with detergent sclerotherapy where there are risks of skin ulcer, hiperpigmentation and even anaphylactic shock. We've been trying to find an ethical way to compare but I can't do a comparative prospective study for the reason I exposed (I have no equipoise because I believe that CLaCS is much more better, faster and safer).
Dear Roberto! Of course, I understand such a problem as you mentioned. I believe that skin necrosis and anaphylactic is of no significance as they are extremely rare. But discoloration is a real problem. Anyway, it disappears after sclerotherapy in some weeks in nearly every patients. If you can not get a control arm with sclero, maybe some of you colleagues practising sclerotherapy can help with it. Of course, study like this will have a lot of limitations but it will be a comparative study. Just a first step to go further.
It is rare Igor Zolotukhin but we run a private clinic that started studying phebology in 1962. It is rare but happens. As we are a private clinic, we look for what is safer and allows the best possible result. Other important point is that brazilian women are picky and in almost all our country we have temperatures above 20C all over the year. Patients want to g to the sun right after treatment. They don't want any temporary pigmentation (not to mention the chance of a skin ulcer). Some of my followers are designing comparative studies and we hope to have some data in the near future.
