We are involved in a telemedicine project in Mongolia which connects doctors from rural hospitals to doctors from the University Hospital in the capital city. What exactly would you like to know?
My work has focused on remote consultation including transmission and real-time processing of physiological data at high frequency. The clinical area is Neonatal Intensive Care and home monitoring. The University Hospital of North Norway operates a service called VAKe where a multi-disciplinary team is called to the emergency dispatch centre at the hospital to participate in a videoconference-based session with any of the local (outpatient) hospitals or intermediary medical centers in the region, when there is an emergency case that needs assistance from the higher level hospital. This service has been in operation for a few years now. Technically, the videoconferencing system is based around a standard setup with one roof-mounted camera above the patient, and an overview camera providing information about who is present in the room and what they are doing. In addition, there is a video feed from the vital signs monitor.
The real success of this system is in the routines and procedures for calling in the multi-disciplinary team and getting them to attend the conference and in the way that the team at the remote end (specialist end) controls the cameras and technical aspects of the videoconference call - in order to keep the load on the team with the patient as low as possible allowing them to focus on the patient.
By using this system, the local hospitals can get access to higher level experts on special cases, improve the selection of which patients to send to the higher level hospital, get support for procedures that can be done locally to prevent the need to send off the patient - if the patient can be treated locally. It is also an effective way of informing the the teams at the higher level hospital about patients being transferred to them.

We use Telemedicine daily in our statewide Stroke Program between rural EDs and our main facilties here in Arkansas. I am going to forward your question to our Trauma people as well, as we I also use this for Trauma consults.
As you know, Telemedicine in an important part of TICs. In our country, Equador, the development of Telemedicine has been limited and small private institutions start some projects about. We have experience in Telehome Care in special in Chronic Diseases (diabetes, hypertension and obesity) in elderly with good results and excellent responses from patients.
