These two are the most common journals. I agree with Nigel that you see more and more papers published in the regular clinical journals. In addition you see an increasing number of telemedicine related papers in IT journals IEEE journals and journals about human interaction
Of course, a lot of journals can be found via Google. But I'm searching for those journals also accepting papers not reporting about revolutionary findings.
The problem I wanted to emphasis is as mentioned in the question itself getting the paper accepted is a bit difficult in higher ranked journals I heard from some colleagues.

You could check national journals such as E-HEALTH-COM which is in German but might be fine; telemedicine is anyway quite often linked to the national / regional specialities.
Obviously it would be easier to publish in the journals that have not been indexed in major indexes like ISI or PubMed. For the same reason, it will be easier to publish in newly established journals as they need enough material (papers in the first or second volume) to apply for indexing.
