The questionnaire was not included, but the authors are on ResearchGate if this looked possible:
Chiang, C., Labeeb, S. A., Higuchi, M., Mohamed, A. G., & Aoyama, A. (2013). Barriers to the use of basic health services among women in rural southern Egypt (Upper Egypt).
There is some confusion in your last reply. There are different uses/types of questions within the research process. For example, researchers (whether they are qualitative or quantitative in their approach) use research questions, aims and/or objectives to state the original purposes of the study.
There is also the use of questions within the data collection stage - such as questions used during qualitative interviews which are typically semi-structured or open questions. Alternatively, questions can be closed/structured, typically used in quantitative research (as Dean mentions too).
Some background reading of introductory research texts should provide useful additional information.
