Are there any sensors to measure different soil nutrients on realtime basis so that the data can be used for efficient decision making? The sensors are required for remote monitoring of soil health.

Manoj , your question is very good , but I am afraid , there is hardly any technique as of now , available to detect the nutrients , either in soil or in plant on a real time basis. Detecting nutrients non-destructively , there are two types of sensors are frequently used viz., optical sensors using reflectance spectroscopy to detect the level of energy in form of reflectance/absorbance by soil particles and nutrient ions and another using electromagnetic sensors using ion selective membranes that produce a voltage output in response to ion activity of selected ions , mainly concentrated on H , NO3, K Na etc . Often , multi-ion measuring system for proximal sensing using ion selective electrodes are also used. I am enclosing some PDFs fro further reading , with a hope , you will find them useful.

I wonder , if there is any such soil sensor right now commercially available which can sense the nutrients directly in field , without bringing nutrients in soil solution ?
There is a long way to go before such nutrient sensors find any precise field utility in real time quantitative estimation of nutrients available in soil . Such sensors are highly sensitive to soil moisture , which will again be hugely dependent on so many soil properties . But surely , some indicative success are reported. I agree with Ahmad on that account .
i hereby add one imp point- specific plants growth symptoms for specific nutrients' deficiency, which are qualitative but useful way

Very often , especially in perennial crops, deficiency symptoms develop quite late , with the result, such fruit crops suffer a lot in terms of productivity , so late that these nutrient deficiency symptoms go simply unnoticed to growers.And therefore nutrient deficiency symptoms are considered as post-mortem diagnosis..
