Can someone help me get any questionnaire available to assess reasons for delayed presentation in leprosy... also please do help how to validate a questionnaire to make it a standardized questionnaire..

This attachment study was performed with a self-designed questionnaire. They have considered the demographic characteristics, delays and the reasons for these delays were explored, following a time line from the first symptom/sign recognised by the patient to the confirmation of diagnosis and effective treatment.
