Infection prevention is a critical element of perioperative quality and safety, underscored by the SARS-CoV-2 pandemic, added the universal rise of antimicrobial resistance doubled the burden.

If you are considering addressing the risks of preoperative infection, the causes and sources of infection must be the starting point. These vary somewhat with the type and site of surgery, but there are commonalities. In a modern setting, theatre hygiene, instrument sterility and clean air are taken for granted, and the major source of surgical site infection is almost always the patient. There is a good system of asepsis and antisepsis in most guidelines, and these are important. Use of prophylactic antibiotics is important in most, but not all, types of surgery, and the choice of agent should be governed by the local knowledge and experience of what is common. Prophylactic antibiotics should be given as one dose only, about 30-60 min before knife to skin, and none postoperatively, to avoid complications and resistance.
