We want to conduct a study on patient satisfaction with outpatient service (like satisfaction with appointment, with nurse, doctors and hospital facilities) in general hospitals. I will be appreciated if anyone can share a standard study questionnaire which was used in others studies?

I have attached a review of general surveys (for overall service contributed by any profession) used in Australian hospitals and I have a few specific outpatient physiotherapy one's if you are interested. The CSQ-8 is a pretty simple one that has been validated

You can observe, use check list or questionnaire for quantitative study but qualitative study or mixed method approach will give you in-depth results.
