Primary syphilis (the chancre) is most often painless and many people in low income countries don't even seek treatment. But secondary syphilis can be distressing, including with systemic symptoms. What proportion of Africans suffering from it seek medical care? I know stats for urethral discharge (~30-70%) but not for secondary syphilis.
There is good epidemiological data from Europe in the 19th and 20th centuries. Try British Army records for S Africa.

Check the Global Burden of Disease study by IHME at the University of Washington. Having said that (I think) you are more likely to get information if you narrow down your search given that there are over 50 countries in Africa with their own statistical systems that tend to differ widely from each other.
