I have a question about the ROB2 tool and quality assessment of the papers for meta-analysis.
The question is about the ROB2 tool question "2.1 Were participants aware of their assigned intervention during the trial?"
My problem is I have a meta-analysis about preterm infants. They are the ones who are in therapy and they are subject to testing after therapy. Parents are aware of the assigned intervention and in most of the papers, they do know what it is. So should I answer the 2.1 question with yes or no?
My logic is that it's a no since the preterm is the one being tested and the little one doesn't know that it even had therapy, and it can't fake the results.

Technically you are correct the participant being the preterm infant is unaware of the assigned intervention but in this case it does seem to warrant some contextual explanations to be accurate in your assessment. What are the measurements being taken, would knowledge (if the participant was able) of the intervention be able to change thE results of the measurement? Does the parents knowledge affect the results? I’d say you are correct to answer the ROB2 2.1 question as ‘no’ but I would take the time to also evaluate the context and subsequent implications to the results.
I'm working with animals in veterinary field where we have the same issues with owners. When the owner does know the treatment the main important thing to look at is if the outcomes of interest depends on them (ex how do you quote quality of life of your animal after intervention). In your case if the outcome are for example in a NICU where the parents do not influence the outcome I would also answer " no" to your question as proposed by Christine Waddell
