Cross and Angelo published classroom assessment techniques in the 1990s I think it went to a second edition. At the back of the book was a teaching goals inventory for faculty I am not sure that it speaks directly to your question but some of the items in it may be helpful in crafting a questionnaire and I'm sure they would give you be willing to give you rights.
