There is a study undertaken by Goud et al (2014) on A questionnaire survey of awareness of physical activity among the faculties of medical college. It can be found on the Journal of Education and Health Promotion.
Are you looking to measure knowledge/accuracy of health benefits, awareness of particular health "facts", personal beliefs in the benefits of exercise ...? The instrument to use depends very much on why exactly you have in mind to measure. A valid" instrument is only really valid if it is suitable for the purpose you need it for. We recently surveyed health professionals on their knowledge of physical activity health "facts" and even then it is difficult because for many facts such as optimum PA for health benefits there is no single correct answer! Also, look for Fiona Bull's paper (maybe two papers?) surveying medical professionals' knowledge and understanding of physical activity information.
