I repeatedly find a minor subset of granulocytes in healthy human donors that stain for MHC-II (HLA-DR) quite above and distinct from the majority of granulocytes.
Are any granulocyte populations known to express MHC-II in healthy humans?

I have not done any of those studies myself, but there are reports of "DC-like neutrophils" expressing high levels of MHC II, although mainly found in disease and not healthy controls (no or little expression). The DC-like phenotype is induced by certain cytokines, in particular IFN-gamma and GM-CSF. There is a nice overview of this topic in a recent review by Takashima and Yao in JLB 2015 (Neutrophil plasticity: acquisition of phenotype and functionality of antigen-presenting cell).

MHCII can be induced in neutrophils and eosinophils. I would put my money on the neutrophils though, as they are more frequent in blood of healthy individuals than eosinophils. MHCII on these cells is induced under inflammatory conditions, that is true. However, in case of local or subclinical inflammation I assume that the donor would be considered healthy.

Reaction was dependent The reaction was dependent on the recognition of allogeneic MHC class I molecules by recipient's CD8 T cells. It gives me reason to believe that similar cell types may appear in the case of viral and intracellular microbial infections.
There is the number of reports when in humans these cells were observed under different inflammatory and autoimmune conditions.
