As many of you, we get involved all the time with issues of sample size calculation. However, besides the use of some software, it is difficult to find a good source of guidance on this critical topic. Are you aware of any good books on this in the literature? Maybe a statistics or epidemiology book that has good chapter on this? Or a book entirely devoted to it?

If you are running experiments where you manipulate some factor or factors and then look at the effect on disease then you may have at least three general methods that will all influence the proper sample size, and all three could be part of one experiment.
1) The experimental design: is this a t-test, linear regression, or some other method?
2) Prevalence: how common is the disease? Are you sampling the population, or selecting only disease individuals from the population? You will need to have enough samples to estimate prevalence with sufficient accuracy to detect treatment differences.
3) Pooling: For rare conditions you might pool samples to more effectively balance the number of required samples with available budget. Here you combine several individuals into a single sample and look at the number of combined samples that test positive versus negative to estimate prevalence. The approach could also be used to test a large number of individuals and then individuals within each positive pool are retested to identify the positive individuals. In any case, pooling will affect sample size.
