Is the coupling of stretching-bending included in the Foppl-van Karman equation for two-dimension?

I am not familiar with the Foppl-van Karman equation describing the large deflections of flat plates, where stretching is induced by bending in three-dimension, but not in two-dimension. I want to ask why the coupling of stretching-bending is not included in two-dimension? Which terms result in this?
