Reversal of cupping has been reported in cases of glaucoma in which IOP has been surgically reduced. If reversal was truly occurring then we should have also witnessed the regaining of nerve fibers, vision and visual fields as well, but none of them is evident in such cases.

When IOP is drastically reduced surgically there is rebound vascular congestion of the disc and narrowing of the physiological cup, which I believe is not a true reversal ( Ref: pseudo-papilledema due to hypotony: Ocular Pathology- Yanoff, Fine 1975; Harper&Row P489).

Why should we even use the term reversal of cupping when it is an established fact that the vision lost in glaucoma cannot be regained? Reversal of cupping has been reported only in glaucoma surgery patients where too much lowering of IOP is a common complication. Reversal of cupping has never been reported in medical treatment cases since the maximal medical treatment of any combination can’t drastically reduce IOP because of built-in safety (auto regulation) mechanism.
