I want to analyse bacterial culture for alcohol detection and quantification. Is it necessary to add internal standard into the culture medium prior to extraction or it can be added into the extracted solvent after the extraction process? Does it make any difference in GC-MS analysis?

As a general rule of thumb for GC-MS, internal standards should be added before extraction. Is there a particular reason for adding the standard afterwards, e.g. are you concerned the standard will be cytotoxic to your cultures?

Yes. Extraction will decrease your recovery so your internal standard should be chemically and physically similar to your GC-MS target. In terms of calibration you will want the ISTD quantitation.
