Everyday more than 1000 cataract surgeries are done through different eye hospitals, surgical mobile camps and private clinics in Nepal. Patients fall in line and their cataracts are removed. Those who are lucky see good, those who are not are told that they had existing problem at the back of their eyes. Are we treating them ?

Absolutely, I would also think that a comprehensive evaluation prior to surgery will be of benefit. We have primary eye care clinics in South Africa that deal with many indigent patients requiring cataract surgery but strict treatment protocols are in place to prevent unnecessary surgery and complications.

Surely Clinical audit for cataract surgery is extremely important. This includes preoperative evaluation- such as examination, type of cataract- density (hardness), presence of any posterior polar cataract, any subluxation, pupillary dilatation and Intraocular pressure to rule out glaucoma. Fundus examination to rule out posterior segment pathology. To rule out or identify systemic diseases such as diabetes and ocular involvement will affect outcome. Accurate biometry is important. Choosing accurate surgical procedure and noting and handling complication is helpful. Postop care and audit of surgical complication and outcome remain very vital. It is must to do audit of preop, operative and postop performance.
You have make distinctions between clinical assessment made by the clinician & the one being done by the service provider (the entity who funds, the service manager, the local insurance body, etc.); & the one being run by broader governing entities (local health authorities, MoH offices, insurance bodies, etc.). The latter two, internal n external audit, can be continuous or periodic. Even for the 3rd one, periodic surveys can be implemented.
Audit is needed for two important reasons: quality of cataract surgery service and cataract surgery indications.
