A study on MI could expand to cover incidence, mortality, risk factors, prognostic factors, survival, complications ....etc. Your questionnaire and the questions it contains will depend on the area of interest you like to study. We found the highest correlation of CHD death was with milk consumption, and the second highest was with sugar. Galactose in milk sugar is a potent glycator. It attaches to LDL, which is then oxidised, and deposited in artery walls. Fructose in sugar is a fairly potent glycator. Also the membrane round milk fat globules causes platelet aggregation, at least in vitro. So I suggest you look at these. However, milk and sugar are often hidden in processed foods, and people do not realise how much of them they consume. Trans fats are also important, and often hidden.
On the protective side, you could look at protection by consumption of vegetables, nuts, seeds, pulses, and fish.
The questionnaire should comprise the following: family history of CAD, diabetes mellitus, hyperlipidemia; smoking; diet; lipid parameters (CHO, LDL-C, HDL-C, TRG, non-HDL, apoB, ApoA1, Lp(a)); history of diabetes and glucose parameters (HOMA-IR index); stress factors.
What do you think about asking questions regarding autoimmune diseases like systemic lupus erythematosus? Might be worth investigating in the questionnaire.

Now it is much clearer that you want to study prevalence ( perhaps you mean incidence), risk factors and mortality. Keep in mind that these are three important areas of interest in myocardial infarction. The handling of these three dimensions depends on whether you are an academic, a postgraduate student or a specialist physician working with the Ministry of Health. A lot of constraints are surrounding such studies. However if you make further clarification on your status I would be very prepared to share you further advice about your proposed research work. Anyhow, the three dimensions can be handled by one big project or three separate specific projects.
