Has anyone ever tried to permeabilize cells (or isolated organelles) without fixation first?

Yes, rarely used. Sometimes after fixation antibodies have trouble to reach the target (say, dynein arms in centriolles), so permeabilizing first for a few seconds, helps with this type of detection.
Yes, that method works. You can permeability first and internalize your material and then fix them for the imaging purposes.
