I have a young 26 year old male patient with active ankylosing spondylitis (predominantly axial, HLA B27 positive). With his current disease activity, he is eligible for anti TNFs. However he was also detected with active multiple sclerosis for which he is on treatment with the neurophysician.

He is on continuous NSAIDs and intermittent IM depomedrol for painful exacerbations. I am considering administration of rituximab infusions as a small study of 20 patients (Arthritis Rheum. 2010 May;62(5):1290-7) did show some promising results in TNF naive patients. 

Any suggestions?
