i have emailed most of the author of the related study but non of them answering my question and non of them willing to share those questionnaires
I do not have a questionnaire related to your subject exactly, but a questionnaire about a very much related subject involving pregnancies. Access to reliable voluntary contraception is the most important factor to reduce maternal mortality and morbidity (I can mail you the references). This helps in postponing, spacing and limiting if so desired. If you could use my questionnaire (see link, open access) for a study I would be very very happy. Also the prevention of unintended pregnancies is for doctors, nurses and midwives (and teachers and journalist via education), the low-hanging climate protection fruit (I can mail you also these references).
