Burnout and perception are behavioral sciences, so I would start searching psych-articles for information regarding the different surveys that can be used. I have attached 1 article and a survey that maybe of some help.

Robbie - Sharon has responded well in terms of measuring burnout. Within the same measurement tool/survey - you would need to incorporate 'perception of levels of care'. That may take you to other tools - and you could then form a hybrid tool of them both. Your demographics will be important i.e. is a particular demographic more prone to burnout - and is there perhaps a correlation between burnout and poorer nursing care outcomes.

Sharon & Dean: I found 2 articles which utilized the Maslach Burnout Interventory (and a Likert scale was used) to determine the frequency of burn/out (these would obviously asked by the nurses). I would also include a Likert scale to address the nurse's perception of adverse effects (such as medication errors) by asking the nurses to rate the frequency of these events using a scale of 0=never .....and 5=every day. Nursing demographics such as age, lenth of nursing employment, area of specialty, and education would be included and listed under the Results section as Nursing Characteristics.

The Maslach Burnout Inventory seems to be a an appropriate valid and reliable tool. Keep in mind that you then have to interpret the results within the context of the areas it was designed to measure (Emotional, Depersonalization and Personal Accomplishment). Look out for skewing of the results related to the context of the setting (acute versus community setting), level of education and years of employment . Here Likert scales may provide more useful information if you look at populations with different years of service where you get really creative (such as asking how perceptions changed when the novice nurse became the expert nurse).
