You can use fuzzy theory for both qualitative and quantitative modeling. The linguistic terms can easily be modelled by fuzzy logic, so if you want to develop a qualitative model, you can prefer fuzzy logic approach. On the other hand, it has already used for quantitative modelling since 1965. I think you can find many text books and papers about samples for those modellings. For questionnare, i suggest likert scale based inquiries instead of binary (y/n) ones. If you use likert scale, you can easily interpret the results using score of each question. Besides, you can convert the likert scale to linguistic terms.
