I did serial dilution plating with different bacteria for same bacteriophage for host range determination and obtained the titres.

Actually this has to be determined by you. Every researcher sets their validation of EOP for different phages and their respective hosts.
Take the optimal host (or the highest value you have) and assume that is 100%. The other EOP's will be whatever percentage less than that.
