In some papers, I saw roughness must high for implants and coatings, but some papers show if it does not have smooth surface means it is not suitable for the medical application.What is the use of roughness in implantations and whether we have to calculate roughness in nm or micro meter?

Well the answer to your question depends entirely on application. What medical application would you use this for?
For implants which must integrate, say into the bone, then a rough surface is ideal, as it allows sites for in-growth from the existing bone. Some examples of this are coatings on hip implants.
For those which we wish to be non-adherent, say for use as a dental material, then generally speaking, a smooth surface would be optimal.
Finally, the roughness required will again depend on the application. For example, the structure of bone is that of a composite, with sizes ranging from the micro to the sub nanoscale (see the attached link).
