There are two opposing effects when considering the flow of RBCs in microchannels. One is the axial migration of RBCs toward the centre of the microchannel. This is mainly the result of the deformability of single RBCs and the lift-force is a function of RBC distance from the channel wall. The second effect is the collective diffusion where RBCs diffuse toward the walls due to cell-cell interaction. The blood viscosity (hematocrit) and flow rate determine the fate of the RBCs in microchannels.

Healthy RBC flow in microvasculature or microchannels is primarily concentrated towards the center of the lumen, owing to their distinct geometry and mechanical properties. This leads to the formation of a cell-free layer near the wall, which is mainly inhabited by WBCs and platelets for better interaction with the walls. This well-established phenomenon is called 'margination' and has several implications in the field of blood disorders as well as drug delivery.
