For the ladder, a common practice is to load 1-5 µl per well. The exact volume depends on the concentration of the ladder and the sensitivity of detection required. Higher concentrations of ladder may allow for smaller volumes to be loaded.

DNA ladder is usually not mixed with the loading dye and transferred directly to the well. For samples, I personally used 7:3 ratio of sample and the loading dye and it gave me good results.
