David Burns, one of the major players in Cognitive Therapy, has two books: "Ten Days to Self-Esteem" (for individuals or group members) and "Ten Days to Self-Esteem: The Leader's Manual." Both are published by Quill/William Morrow. You might find these books helpful.
I recent book that refers to protocols overcoming low self esteem and achieving lifelong self esteem (4 step programme) is written by Leslie Sokol and Marci Fox, published by Penguin Group is called "Think confident , be confident". I hope it helps. Leslie is a good friend of mine who is a very competent therapist. (Another book is by Melanie Fennell and it is titled as "Overcoming loe self esteem").

I would recommend you the Competitive Memory Training for Low Self-Esteem (COMET) by Kees Korrelboom and colleagues. It has a good evidence base and it is easy to use if you have got some experience in CBT. I Think you can get fulltext papers via Kees Korrelbooms Researchgate site or if you write him a message.
Is Low Self Esteem a condition? I used to think that it was but having worked with Chronic and Long Term Depression, I now believe that this is an aspect of Dysthymia...in that people function but not that well based on a range of activated beliefs through their environment. This renders them vulnerable to episodes of low mood based on contextual factors and mood increases when Rules For Living have been effectively sanctioned and used again to alleviate the low mood.
LSE isn't disorder based on any classification, but It is transdiagnostic phenomena that can be share in many disordres such as dysthymia. In CBT approach, there are protocols that focus this part of psychopathology particularly.
