How would you interpret the following sentence -
" Global median rate of community residential facilities is 0.008 per 100000 population, there is presently a median of zero facililities per 100000 in low and middle income countries , as compared to 10 facilities per 100000 population in high-income countries."
Here, what will be the interpretation of global median rate and median of zero facilities?

Suppose there are 100 low and medium income countries and 50 of them have zero residential facilites/100000 ie 50% of sample so median value among low and medium countries is zero .
Suppose there are 30 high income countries and 15 have residential facilities less than 10/100000 ie 50% sample so median value is 10/100000.
So, the interpretation of the sentence suggests that there is a significant disparity in the availability of community residential facilities between high-income countries, where there are on average 10 facilities per 100,000 people, and low and middle-income countries, where there are none on average.
The previous answer is describing a mean or average-not a median.
The median and mean of a distribution are not the same. While for symmetrIc distributions they will be close in value, for asymmetic distributions they will be different.
