How do I run a meta-analysis of studies that used different scales for scoring/measurement?

I am performing a meta-analysis to see the effect of the intervention on the repair of cartilage. I have two sets of data from the included studies.

Category 1:. lower score favors intervention

Category 2: higher score favors intervention

1. Can I run the two categories separately and perform subgroup analysis for each category in case of high I2?

2. Should I use SMD or MD? because even in each category there are variations in the scale used for scoring.
