Presence of clue cells in vaginal swab is a good indicator of vaginosis. In vaginosis only a few epithelial cells are clue cells which are loaded with bacteria on their surfaces. Why are some vaginal epithelial cells converted into clue cells but not others?
Numerous bacteria, including gardnerella have adhesins that promote sticking to themselves and surfaces. Hovever.,most important is the formation of a biophilm in which bacteria feel happy according to their ideal redox potential inside the biophilm. This explains also why eradication of certain anaerobes with metronadazole does not garantuee complete cure and increased risk of recurrences, as long as the biophilm is not destructed and stays in place. Thanks Gilbert. In the picture you can see that one epithelial is loaded with bacteria ( clue cell) but two other nearby epithelial cells escaped. What is the reason?
I also found this type of clue cell in some cases of prostatic smear. Which bacteria should be involved here?

this is correct. In partial (incomplete) BV (is NOT synonymous to 'intermediate flora') there is only a limited number of clue cells positive, but in full blown BV it is often clode to 100 %. This also depend on how superficial the epithelial cells are. Some of them originate from the second or third superficial epithelial cell layer, or are called intermediate cells (again not to be confused with intermediate flora) or even parabasal cells. All of these will of course have no bacteria on their surface.

Nugent score is a very simple evaluation method done on HVS using gram stain. The results would really aid the clinician to on the degree of BV.
Culture: i think we are ignoring as it becomes very easy for us to identify the causative for BV and thereby antibiotic sensitivity.
