There ways to develop questionnaire on HR and technology:

1. Search papers on this subject matter and read carefully

2. Some papers may highlight some scales, with permission and by acknowledging it, you can use

3. You can search scales by using google search engine

4. You develop your own by consulting research papers

5. I have published few paper on HRIS, please consult these on RG

6. Etc. ...!

A general way of tackling a big / vast / broad problem is to divide it into small units or parts, wherein the summation of those parts will be equal to the whole problem. This will effectively simplify the whole problem.

HR is a vast subject. In fact, on HR Degree / Diploma courses are conducted by various institutions. The requirement is, therefore, to determine which unit / units of HR  you would be considering for your research work. Those units can be further divided into sub-units.

For elucidation, let us take the example of Appraisal. There can be many sub-units or points of consideration in this, based on which such a process is normally carried out. These could be (a) Technical Knowledge (b) Self-Motivation (c) Ability to motivate people (d) Discipline (e) Ability for Team-work (f) Leadership Qualities (g) Vision etc.

The next step would be to decide how these units and sub-units are to be tackled and the questionnaire to be made.

A general comment on formation of  questions could be the following. For getting precise answers, the questions should be focused  and not ambiguous.        

It is not a must to create a scale in your proposal. If you just focus on literature review and the method that you adapt, this is better; because lecturers consider your literature review, method and research plan in your proposal. You need to clearly indicate the purpose of your research and how you make your countribution to the literature or practices. I hope this is helpful, because don't waste your time for creating a scale for PhD proposal.
