I have worked in a hospital where the outpatient load is about 1000 patients per day. Categorization of patients and setting up specialty clinics where patients are further split into manageable groups was one of the strategies we adopted to clear the patient load..But this needs lot of support to educate the patients to go to a particular area and repeated reinforcement..The management needs to provide suitable waiting area and patient education material so that the patient is engaged during the waiting period.
I would recommend that you develop a survey of question which you could administer as follows:
First, develop a survey to assess the responses of ancillary staff find out about patient waiting, common emergencies and frequencies. You can add other question related to the variables to test.
Second, if they will allow it, develop a survey for emergency room patients related to their concern and emergencies.
You can start with a local hospital, by writing a letter to determine if they would allow you to conduct your survey.
I have not conducted any study, but on the basis of knowledge I have gained as a student of hospital management. Emergency of the majority of the tertiary level hospital is overloaded. Such hospital catered large number of emergencies cases. For this, we can categorize the patients being dependent on the type of emergencies for a designate clinic with trained staff. Such clinic can be located in various parts of the city in a satellite hospital. Here we can use triage for categorization of the patients. In this way, we can decrease the load of patients in emergency of the hospital.Motivational & well trained staff should also be criteria for the appointment in emergency
Crowding problem in hospital emergency department is mostly related to hospital bed availability. Effective bed management process can reduce the emergency department crowd.
'Lean Sixsigma' method is one of the quality management method you can think about to reduce the crowd. During my MBA in Hospital t and Health Services Management, I did a project to explore the feasibility of 'lean sixsigma' method in hospital bed management process. Have a look in it. A graduate student of mine and I analyzed ambulance log data and hospital ER statistics to determine the effect of ER capacity on ambulance crew and patient waiting times. Some variables of interest were: 1) waiting time from arrival to admission to ER bed; 2) ambulance diversions due to ER capacity limitations; and 3) ER capacity measures. Ambulance log data should be available from the emergency transportation provider and hospital ER measures are usually avialable form the ERs.
