In my opinion, there is a big problem with the use of guidelines in the field of rehabilitation and physical therapy in Ukraine. There are no currently common education standards for physical therapy specialists and standards for the provision of rehabilitation at the state level. Most likely that in the near future, a professional organization Ukraine Physical Therapy association will develop and recommend clinical guidelines for specialists.
Following guidelines would be good practice, if we can trust that their development is based on the best available evidence. Many guidelines, such as most of the Cochrane reviews (SR) and meta-analyses (MA), are based exclusively on RCTs. All other types of evidence are systematically withheld from these analyses. Therefore the recommendations in such guidelines cannot be trusted as being a sufficient source to inform medical practitioners.
Blindly following MARCT-guidelines is dangerous for the quality of care, hence for the patient. Guidelines are NOT a substitute for critical thinking nor for using common sense.
