We did not find any "question&reply" application allowing follow-up questions not requiring any IT-knowledge. We needed anyone to create forms as wished with all complexity without someone else to be involved. Neither we found a tool to gather type based replies for further automatic processing without limitations. Our goal is to create questionnaires and collect valid replies. Since we did not find anyone else to offer this we developed this kind of application ourselves. And the resulting application became a great way to work with. So what I would like to know is what do you use to create questionnaires and to collect replies on a scientific base? Is there any other application out there?

Hello Peer, I am very interested in using your application and providing feedback. I run a small team of social science researchers from Latin America, and this would greatly help us build our surveys, specially for follow up questions.
Since we're a small team, we use either surveymonkey, or, for smaller samples, we use Google Forms, but honestly, but we always encounter a problem with either of them. Working with single-choice questions is easy, but when doing multiple-choice questions, we always run into the same problem:
When you export the data into a csv/excel file, it always crams everything into a single column, which you then have to separate, and, when you import that data to a statistics software like SPSS, or a qualitative analyisis software like Atlas, there's a lot of workarounds to do in order to make the data fit (so much that sometimes the survey needs to be changed to accomodate those limitations). It would be fantastic if your application could rid of of these headaches, and if the interface is user friendly, I'd totally recommend it.
I have encountered similar problems trying to use SurveyMonkey and other commercially available Web-based software packages to generate questionnaires in the area of sexual behavior and mental health including recreational drug use, and would be very happy to try out your group's solution using your application in ongoing longitudinal Web-based surveys. In addition to the problems mentioned by both peer and Joseph, since the data being collected by me is very sensitive and therefor subject to privacy rules of both HIPPA and the NIH Office of Research Involving Human Subjects, any web-based survey application must be HIPPA compliant, which is a difficult problem when using any of the existing survey generating software applications which involve transferring the data between the application's database and internal research databases used for analyzing longitudinal observational data. Hopefully your solution, Peer, gets around that problem by allowing the researcher to maintain control and confidentiality of the resulting database. Thank you very much.

Joseph, are you asking for a one-person login to the application hosted by us?
David, are you asking for a local server ("enterprise-server") on site in order to maintain all data strictly to your group (avoiding the www)?
I guess best is to start with a "Welcome-System" online and you'll see how you like it, all right?
We are Brownfield research team in Northwestern polytechnical University. Unluckily, we dont have access to use Google or Google docs here in China. Our team use "www.gojump.com" to collect data. Its interface is too easy and data can be exported to rigrous statistical softwares easily.
