Excess Mortality in Cost-effectiveness analysis?

Dear All,

How we can know whether we have to consider additive or multiplicative hazard model to calculate excess mortality (force of mortality) due to disease in decision analytic modelling for cost-effectiveness analysis?

I think it depends on whether each case of mortality is dependent or independent of the others. If they are dependent, either on each other or on a third variable, then use a multiplicative hazard model. If they are independent, then use an additive hazard model.
