In my country, almost all universities have an Ethic Committee that provides this kind of approval regardless of the participants' affiliations. So I guess your unversity should be the one to get the approval unless you have another institution that deals with this matter. Hope this helps.

In fact, in some universities, you can´t carry out any research without approval of its ethics committee, given that the universities protect their own reputational image, that can be damaged if a research doesn´t attend the values and principles that the university declares to protect.

It is for your and your respondents' best interest for you to secure a research ethics clearance from your university. It will save you from a lot of potential troubles.

Un saludo mencionas se "realizó" eso quiere decir que primero hiciste la encuesta y después solicitas quien te da la aprobación? Es un proceder no dentro de los lineamientos de la investigación científica asi esta sea cualitatitativa. Se observa, se plantea hipótesis, se fundamenta, se hace el protocolo , se desarrolla. Pero antes de iniciar el desarrollo de un protocolo se debe pasar por las instancias ética o institucionales que se requieran quienes darán los lineamientos éticos entre esos como manejarías la propiedad de los datos, si es o no población de riesgo o especial, etc... que entre otras debe estar normado .

Creo que tu experiencia nos recuerda que antes de iniciar cualquier investigación de carácter clínico asi sea una encuesta debe pasar primero por el comité correspondiente, no después de realizar buscar la aprobación de un comité.
