Several survey questionnaires can be suitable for use in different populations, times, locations etc. Using an identical survey allows direct comparison between groups. My question does the re-use of a survey questionnaire published in an open access article represent a breech of copyright and thus need special permission from the author OR it can be used without permission and simply attributed to the author(s) citing the publication in the methods and references and perhaps in the acknowledgements section?.

Questionnaires are subject to copyright. This also applies if they are publicly available. So you need permission from the authors to use them, unless they have given general permission to reuse them.

Further to the comment by Wolfgang R. Dick I note that in many instances (for almost all of my articles), I signed the copyright transfer to the journal/publisher so I think they might be the "owners" that you have to contact in this situation.
