I am looking for a self-report questionnaire that was specifically developed for the assessment of cognitive complaints experienced by patients that experienced a traumatic brain injury.
NINDS Common Data Elements is usually a good place to start your search (https://www.commondataelements.ninds.nih.gov/TBI.aspx#tab=Data_Standards). Under the Patient-Reported Outcomes section you would find several relevant measures. I'd probably opt for the TBI-QOL - it also assesses several other domains apart from cognition.
You might want to take a look at this publication: Ponsford et al (2012) Fatigue and Sleep Disturbance Following Traumatic Brain Injury- Their Nature, Causes, and Potential Treatments. Journal of Head Trauma Rehabilitation. 27(3):224–233. They report a few measures of fatigue in TBI, so you can review those, particularly the Causes of Fatigue Questionnaire (COF- Ziino and Ponsford, 2005) which includes ratings of fatigue associated with cognitive and physical activities.
You may have a look at Multiple Ability Self-Report Questionnaire (MASQ) to assess cognitive functioning and Fatigue Symptom Inventory (FSI) to assess level of fatigue.
