An 18 year old male with juvenile Diabetes develops severe rhabdomyolysis upon heavy exercise and has been hospitalized many times. He hydrates often during exercise.
I don't have a great article to send you. However, this has been discussed a few times in the setting of combined t1d and MCAD deficiency. You may consider checking for this as there is genetic testing.
Rhabdomyolysis has been reported in DM due to Hyperosmolar Hyperglycemic state which occur T2DM . In type 1 DM , it can occur , if DKA is combined with hyperosmolarity ( Osm > 320 ) . In addition , hypokalemia & hypophosphatemia might contribute . It would be valuable , if the laboratory data of this patient are available to discuss the mechanism of Rhabdomyolysis . It is essential to determine in this T1DM patient , whether this is due to trauma of exercise , or non traumatic due to metabolic complications or a combination of both .
