I'm trying to track down a questionnaire which originated with the International Federation of Medical Students' Associations. It was used in a couple of studies but the original seems to have vanished from the IFMSA website.

Yes, I reached out to the President of IFMSA, Omar Cherkaoui ([email protected]), who kindly provided the ISSH questionnaire attached (from 2007) as well as two additional reports, if you are looking for more information.
Omar is very much interested to know what you plan to do with the ISSH questionnaire. Would it be possible to keep us in the loop?

Yes, but too late. Yesterday I contacted Omar Cherkaoui asking him for information about where to find the Questionnaire since all links pointing to it are wrong. Omar was so kind to send me the document and was very interested in your project. I see Dr. Salerno already has an answer.
Antonio and Principessa! Thank you both very much. I shall write to Omar to fill him in on the project we are planning. I was surprised that the questionnaire, after some initial interest, seemed to have fallen off the map.
