Please clarify , what do you laser? The pelvic floor is complex and endoscopic or laparoscopic or open laser could target may structures what are you proposing?
Thank you for replying. I am uncertain as to your target for laser. What type of laser do you use and what target? The vulva and perineum have significant structures that can be targeted and an internal probe suggests intravaginal use. Again please clarify what structures you target? nerve muscle insertion? What laser subtype ? this determines penetration and what is the definined purpose of the laser? Are you destroying the sensory elements or denervating muscular elements or are you targeting the muscle fibres or tendinous insertions or origins. Without the delineation of the equipment and target it would be difficult to evaluate or comment on the efficacy
Effect of Vaginal Stretching and Photobiomodulation Therapy on Sexual Function in Women With Pelvic Floor Myofascial Pain - A Randomized Clinical Trial
the use of photobiomodulation which falls within the infrared spectrum and not laser is clear from the articl3e however I am still not understanding the use of laser as the original question states? No clarity as to the type of laser used nor the target of the laser which as we use it in tissue and urolithiasis seems destructive at best?? Holmium Neodymium and thulium YAG being employed most commonly
Thanks Cindy for answering. We finished the literature review already. The photobiomodulation (or LLLT) is the target for our group of physiotherapists; we found good evidence to support it's use. We work with chronic pelvic pain, including vulvodynia patients. We were interested to know which brands are better and which machines have an internal probe. Thanks for your help Cindy!
Thank you for this. Please note that photobiomodulation is not laser per se . In urology the use of the neodymium YAG , holmium YAG and Thulium YAG together with the side fire and green light laser which have well defined and extraordinary use. Photomodulation is well described in dermatological use. To ask about pelvic floor which anatomically is also a rather complex anatomy with a number of target from neural tissue , muscular and then the fibrotic bands and tendineous portions must be clarified. Therapy is designed to increase vascularity to generate neovascularisation , destroy nerve fibres etc . The correct descriptors of what the therapy entails remains central to the understanding of the postulation being explored.
